Community of Practice in Implementation Science for Geriatric Medicine

About the Community of Practice

The Community of Practice (CoP) in Implementation Science for Geriatric Medicine is a national platform that brings together clinicians and researchers to strengthen implementation science capabilities, support the translation of evidence into practice in the care of older adults and build collaborative networks across healthcare clusters.

The CoP is designed as a learning community that extends beyond the traditional seminar series. It provides opportunities for knowledge exchange and practical learning among clinicians and researchers.

Format

Sessions are organised quarterly to explore how evidence can be put to work across two domains central to the care of older adults:

1. Preventive and care models in geriatrics: exploring how evidence-based approaches can strengthen the prevention, assessment, and management of conditions common in older adults

2. Digital health and AI solutions to support the care of older adults: examining how emerging technologies can be meaningfully integrated into workflows to support the care of older adults.

The Knowledge to Action (KTA) framework guides how our sessions are structured, helping participants move from understanding evidence to applying it meaningfully in their own clinical settings. Anchored around the KTA model, the CoP sessions encourage discussion and exchange. Participants are guided through the journey from evidence to practice and supported in adapting and applying that evidence within their own settings.

Sessions are built around the practical building blocks of implementation:

1. Stakeholder engagement

2. Needs assessment

3. Selecting and tailoring implementation strategies

4. Developing evaluation plans

5. Cost-effectiveness analysis

International collaboration

The CoP is established in collaboration with the Knowledge Translation Program (KTP) at St. Michael's Hospital, Canada.

Professor Sharon Straus, Director of the Knowledge Translation Program at St. Michael's Hospital is a globally recognised leader in knowledge translation and serves as the CoP’s international expert advisor.

From Evidence to Action: Transforming Advance Care Planning through Implementation Science in Singapore

Friday, 21 August 2026 | 12:30 – 2:00 PM | Microsoft Teams

This session explores how IS principles were applied to develop Singapore's first national ACP Quality Framework, and what this means for translating research into practice.

Participants will gain practical insights on defining and prioritising implementation problems, navigating barriers, and engaging stakeholders, using the ACP Quality Framework as a live case study.
