Objective
To improve prescribing for older adults with multimorbidity attending hospital outpatient clinics by combining evidence synthesis with expert opinion elicitation to inform development of a care intervention. The specific aims to achieve this are to:
Identify key barriers to appropriate prescribing for older adults in the outpatient care setting.
Identify useful medication prescribing guidelines for older adults with multimorbidity.
Identify effective care interventions that can be implemented to facilitate appropriate polypharmacy in this setting.
Adapt a risk stratification model for potentially inappropriate prescribing among older adults with multimorbidity attending hospital outpatient clinics.
Develop a physician-pharmacist collaborative care intervention for older adults receiving hospital outpatient care based on first 4 aims.
