Objective
Cognitive impairment is not detected in substantial proportions of older people at primary care who have the condition. Early detection allows for earlier diagnosis and appropriate intervention, education, psychosocial support, and engagement in shared decision-making regarding life planning, financial matters and healthcare. This project aims at early and systematic detection and intervention of cognitive decline and dementia in the population by implementing and evaluating the feasibility of a scalable preventive community-based intervention programme to improve and maintain the cognitive functioning in seniors.
