Health Outcome & Process Evalation on Project Tri-generational Homecare (HOPE @ Trigen)

Objective

​Evaluates the impact of a multidisciplinary university student volunteer healthcare team comprising of Medical, Nursing, Pharmacy, Psychology and Social Work students (TriGen Homecare) on community-based patients under the Ageing-In-Place (AIP) programme with chronic medical conditions in improving their self-efficacy in chronic disease management and quality of life (QoL) and to examine the impact on hospital admission and length of stay (LOS).
