Multi-site Evaluation of the geriatric services hub (GSH)

Objective
One in four Singaporeans will be 65 years or older by 2030. With this, increasingly frail older adults with more complex needs will be encountered by health services. Thus, a recent MOH initiative supports development and implementation of five Geriatric Services Hubs (GSH) towards the vision of providing integrated care for frail older adults in the community. The study aims are:
To evaluate the effectiveness of GSH in improving health, experience of care, health services utilisation, social and economic outcomes.
To evaluate effectiveness of the care model in the process domains of patient-focused care management and coordination/integration of care.
Date collected and analysed will generate a knowledge translation platform that documents the different mechanisms and impacts at different sites.
