The Advance Care Planning (ACP) programme in Singapore was launched in 2011. It was modelled after the ‘Respecting Choices’ programme in the USA. Since then, thousands of ACP conversations have taken place to allow individuals to make their wishes known and honoured. It is now an opportune time to review the evidence of how the ACP programme has been faring and impacting the lives of patients and family members.
The ACP programme is highly complex and faces many challenges that have to be navigated carefully in order to make a difference in the lives of those who are near the end of life.
The objective of this knowledge exchange platform is to highlight the latest evidence and understand how this could be translated into practice. In addition, it seeks to provide updates and discuss possible ways to enhance the many aspects of the ACP programme such as the training of facilitators, community outreach, administrative policies, etc.
