Patient-Centered Medical Home (PCMH) is a model of healthcare that aims to deliver integrated, patient-centered care for complex needs patients. The 1st PCMH demonstration in Singapore launched in November 2016. Implementation was guided by principles including comprehensiveness, coordinated care, patient-centeredness, accessible services, and quality and safety. We aimed to investigate the impact of this care model on quality of life (QOL) and patient activation.
