Ageing in Place Community Care Teams Evaluation (AIP-CCT)

Objective

​To evaluate the effectiveness of AIP-CCT programme among resource-intensive patients. The project will plan to have three phases where Phase 1 will pilot a community pharmacy service, as well to conduct a needs assessment through retrospective analysis of administrative data on hospital utilization and health outcomes. Findings from Phase 1 will inform subsequent phases involving risk modelling followed by process and outcomes evaluation.
