Person-centred Care (PCC) should be considered as the gold standard model of care for the older persons. However, it is not widely practised in many settings because there is a lack of understanding of what it means and how to translate into practice.
Traditionally, intermediate and long-term care (ILTC) facilities in Singapore have adopted an institutional and medical care model transposed from the acute care setting, which emphasises and focuses on safety and risk mitigation of patient care. Today, PCC is believed to be synonymous with high-quality care that maintains the dignity, well-being and personhood of residents. As such, transformation efforts to promote PCC in the ILTC settings should be evaluated and backed by good scientific evidence that is translatable and contributes to upholding the dignity and autonomy of residents.
Geriatric Education and Research Institute (GERI), in collaboration with Agency for Integrated Care (AIC), is organising this webinar with the primary aim of exploring available local research evidence in conjunction with relevant updates that could help advance the adoption of the PCC model of care.
Do join us to hear from our distinguished speakers and panellists who will share their wealth of knowledge, experience and expertise from the real-world.
